Liability Release
I, the undersign, do hereby consent to the participation of myself in the scheduled serve team events of Epicentre Church. I hereby release, discharge and/or otherwise indemnify and hold harmless Epicentre Church, its affiliated organizations, sponsors, volunteers, employees, and associated personnel against any claim or written demand by or on behalf of the registrant as a result of the registrant's participation in the serve team event. I give consent for myself to receive emergency medical care. This care may be given under whatever conditions are necessary to preserve life, limb, or well-being.
